GLP-1 Medications and Your Other Prescriptions: an Absorption Audit You Can Run Yourself
Slowed gastric emptying is the mechanism behind most of what a GLP-1 does to appetite, and it is the same mechanism that changes how every pill you swallow gets absorbed. For most drugs that does not matter. For a specific short list, it does.

The conversation that starts a GLP-1 prescription covers the injection, the titration, the nausea and the cost. It almost never covers the other eleven things in your medicine cabinet. That omission is understandable, because for the large majority of medications the interaction is real, measurable in a pharmacokinetics laboratory, and clinically irrelevant. But the exceptions are not obscure, and one of them is among the most commonly prescribed drug classes for women of reproductive age.
The original element in this piece is a three bucket audit you run on your own medication list, with the sorting rules stated first so you can apply them to anything, including drugs invented after this was written. It is not a list of interacting medications. Lists go stale. It is the method for producing your own list, which is more durable and considerably more useful in an appointment.
The mechanism, in one paragraph. GLP-1 receptor agonists slow gastric emptying. That is not a side effect, it is a substantial part of how they work, and it is why fullness lasts. A pill swallowed into a stomach that empties more slowly reaches the small intestine, where most absorption happens, later and more gradually. For a drug whose effect depends on total exposure over a day, arriving late changes very little. For a drug whose effect depends on reaching a peak concentration, or one with a narrow margin between effective and toxic, arriving late and flatter can matter. The pharmacokinetic behaviour of semaglutide and the implications of that behaviour for co administered drugs were characterized early (Clinical Pharmacokinetics, 2018).
The reassuring part is that the systematic evidence, taken as a whole, is undramatic. A 2024 systematic review of drug interactions between GLP-1 receptor agonists and oral medications found the interactions to be largely modest in magnitude and rarely of clear clinical significance (Drug Safety, 2024). That is the correct headline. The audit below is not about panic, it is about finding the handful of items where modest is not good enough.
The sorting rules. Take your actual list, including over the counter medicines and anything you take intermittently, and ask four questions of each item.
First, is it swallowed. Injections, patches, inhalers, implants, intrauterine devices and anything absorbed through the skin or a mucous membrane are unaffected by gastric emptying and go straight into the no concern bucket. This alone will clear most lists.
Second, does its effect depend on a peak or on total exposure. A painkiller taken for an acute migraine, a rescue medication, or a sleep aid depends on reaching a concentration quickly. A daily blood pressure medication or a statin depends on total exposure across the day. Peak dependent drugs are the more sensitive category.
Third, does it have a narrow therapeutic window. Meaning, is the gap between the dose that works and the dose that causes problems small, and is it monitored by blood test. Thyroid replacement, anticoagulants, antiepileptics, lithium, and immunosuppressants live here. These are not necessarily affected, but they are the drugs where a modest change is worth measuring rather than assuming.
Fourth, does its dosing carry a fixed timing relationship with food or with other medication. Anything you were told to take on an empty stomach, or thirty minutes before eating, or four hours apart from something else, is a drug whose absorption someone has already decided is timing sensitive.
Bucket one, documented and label level. The clearest example, and the one most often missed, is oral hormonal contraception with tirzepatide. The prescribing information for tirzepatide advises patients using oral hormonal contraceptives to switch to a non oral method, or to add a barrier method, for four weeks after starting and for four weeks after each dose increase (FDA prescribing information). This is a labelled instruction, not a theoretical concern, and it does not apply identically across the class, which is precisely why it gets lost. A 2024 review specifically examining the impact of tirzepatide and GLP-1 receptor agonists on oral hormonal contraception lays out the difference between agents (Journal of the American Pharmacists Association, 2024).
The practical consequence is that the four week window recurs. Every dose escalation restarts it, which means a patient on a slow titration may be in and out of that window repeatedly over several months. That is not intuitive from reading the instruction once.
Bucket two, plausible and unstudied. This is the bucket the audit exists to populate, and it will look different for every person. It is any drug of yours that answered yes to question three or question four above and has not been specifically studied with your GLP-1. Thyroid hormone taken fasting each morning is the archetype. The interaction is not established, the timing sensitivity is, and the correct response is not to stop anything but to tell the prescriber that both are on the list and to ask whether existing monitoring intervals should be tightened around titration.
The general principle for this bucket is that the risk window is not the steady state, it is the change. Bucket three, no meaningful concern. Everything not swallowed, everything whose effect depends on total daily exposure, and everything with a wide safety margin. Most of a typical list ends up here. Statins, most blood pressure medications, most antidepressants, metformin, and the great majority of routine prescriptions belong in this bucket, and moving them there deliberately is what makes the audit useful rather than anxiety producing.
Two practical additions. If constipation is significant for you, note that it compounds the same mechanism further down the gut, which is covered in GLP-1 constipation and gut effects. And if pregnancy is a possibility in either direction, meaning you are avoiding it or seeking it, the contraception point above is not a footnote, and it sits alongside the wider planning considerations in GLP-1 medications and pregnancy.
What the studies do not tell you. Almost all of the interaction data comes from single dose pharmacokinetic studies in healthy volunteers, usually at a fixed GLP-1 dose, usually not during titration. That is close to the opposite of the real world scenario, which is a patient on a changing dose, with variable nausea, variable food intake and multiple co administered drugs. There are essentially no outcome studies asking whether these pharmacokinetic changes produce clinical consequences, with the contraception question being the notable exception because the outcome is unambiguous and consequential. So the honest summary is that the mechanism is certain, the magnitude is generally small, the population studied is not the population taking these drugs, and the one place where the evidence is clear enough to act on is written in a label most patients never read.
Bring the list. Not a summary of the list, the actual list, including the things you take occasionally. Sorting five minutes of it at the start of a titration appointment is worth more than any amount of searching afterward.
